Forms Required for Registration:

1. Birth Certificate
2. Social Security Card
3. Proof of address: Must provide two (2)
    (Mortgage bill & Electric bill (FPL Bill Only)
4. Health requirement: Must provide both forms.
    A. Student Health Examinations– DH 3040 (YELLOW)
    B. Florida Certificate Immunization– DH 680 (BLUE)
5. Private school students MUST bring grades.
